Defender hooks

Runs `ccpp defender hook` after every tool call, so each result is scored before Claude reads it.

A `PostToolUse` hook with a 15-second timeout. Results of tools that echo Claude's own writing or your answers (Write, Edit, AskUserQuestion…) are not scored; `defender.ignoreTools` changes the list.

Install

Installing the defender plugin installs this component too.
